Intake Coordinator

Department: Abraxas Youth & Family Services
Location: Chicago, IL

Summary of Position: The Intake Coordinator shall support the Intake/Outpatient Clinical Supervisor in the responsibility of assuring the facility’s Intake operations are functioning smoothly. They will serve as the gatekeeper of the Intake Department, managing the flow of patients during Intake hours. This will entail assisting with all parts of the Intake process to include, but not limited to, assisting patients with starting the Intake process through the electronic app, tracking all patients coming into the program, being the point of contact for referral sources, and completing and distributing necessary court documents. They will also keep all data for admitted and non-admitted clients to include walk-in referrals, scheduled patients and Stabilization and Transition Unit referrals and admissions. In addition to tracking admissions, they will also track discharges to ensure we are scheduling the proper number of clients on a daily basis to maintain budgeted utilization. They will also track all those turned away and the reason(s) why and help each client understand what they need to do in order to come back and be admitted into the program.

In addition to the Intake responsibilities, they will assist the Outpatient team with reports and data management.

Essential Responsibilities:

  • Possess a working knowledge of the Intake process.
  • Support the Intake Counselors through appropriate scheduling, monitoring patient flow, and assisting patients through the process.
  • Support and encourage the client and assist in the Intake process. Understand client expectations and work with the client to assist in their admission.
  • Daily, weekly, and monthly data collection and reports submitted to the Admissions & Customer Relations Department.
  • Assist in training Intake staff on the new electronic paperwork/processes.
  • Serve as liaison between Southwood Interventions and The Courts for Court involved clients.
  • Serve as the on-site representative for referring agencies.
  • Work with the Liaison Team to schedule intakes and inform them of open beds and discharges.
  • Review every client AFD daily after completed by the Intake Counselors. Make any necessary corrections and submit all AFDs for processing.
  • Work with the Intake Supervisor and/or Director in assuring coverage is provided when intake staff are out for vacation, sick, or any time off requests.

Minimum Education & License / Credential Requirements

  • High school diploma or GED and twenty-four (24) credit hours of college education or accredited training program in substance abuse counseling and/or other human services area or at least six years (6) experience working at Southwood Interventions with at least one years' experience in the Intake Department.
  • Higher education given preference.

OR

  • Certificate in Addictions (CADC) or clinical licensure in IL as LPC or LSW

Any new professional staff who do not have an IAODAPCA CADC or above certification when hired or first moved into this position must:

  • Obtain an IAODAPCA CADC or above certification (LSW or LPC) within two years after the date of employment; and

Other Hiring Requirements

  • Ability to promote positive behavior and manage resistant behavior.
  • Physical ability to safely and effectively manage aggressive behavior.
  • Assertiveness, confidence and leadership abilities.
  • Effective oral and written communication skills.
  • Effective time management skills.
  • Must maintain a valid driver’s license.
  • At least twenty-one (21) years of age.
  • Criminal clearances (State & child clearances and, if necessary, FBI).
  • Drug screen.
  • Ability to work with computers and the necessary software typically used by the department.
