Founded in 1985 as a subsidiary of the organization then known as Church Home Foundation, Conexio’s original mission was to help older adults with lifelong histories of psychiatric hospitalization to transition into the community. The Community Case Manager is a member the clinical team that provides compassionate care and assistance to persons who struggle with intellectual, developmental, and behavioral health challenges and require assistance to achieve their fullest potential. The Case Manager provides the assistance necessary to allow the clients we serve to live independently and ensure their wellness, health, and safety. The Case Manager, in collaboration with an interdisciplinary team, provides a wide range of services to meet the environmental, physical, psychological, and personal needs of the persons in our care. Duties of the position:

  • Professionally participate in team conferencing to create and implement care plans for clients
  • Maintain a therapeutic alliance with clients
  • Refer and link clients to all needed services provided by an outside agency
  • Coordinating independent living assistant, medical and vocational services
  • Work with community agencies to develop needed resources including housing, employment options, and income assistance
  • Provide support and consultation with the client’s family
  • Provide in-home services including ADL training, medication assistance and training, social support, and home management assistance; providing supportive counseling
  • Document and keep other staff, clinical coordinators, and Program Director informed of all changes in the consumer’s mental or social status
  • Carry primary or secondary cell phone as assigned and responds to after-hour and emergency calls in accordance with program directives
  • Provide education and work as an advocate for persons served in the community

Required Education and Qualifications:

  • Bachelor’s Degree in Behavioral Health, Psychology, Health Behavior Science, or related field
  • Experience working with adults with severe and persistent mental illness and/or substance use disorders
  • Supervisory experience preferred.
  • Valid driver’s license in good standing with at least 3 years minimum continuous driving experience with no serious traffic infractions.
  • Must receive and maintain a satisfactory criminal background investigation report?

Additional Qualifications:

  • Computer knowledge of MS Office, and other HR applications is a plus, not required.
  • Possess strong verbal and written communication skills.
  • Must be proactive, organized, and possess great follow-up skills.
  • Must have the skill/ability to collaborate, partner with, and provide exceptional customer support to a variety of affiliate partners within the organization.
