Founded in 1985 as a subsidiary of the organization then known as Church Home Foundation, Conexio’s original mission was to help older adults with lifelong histories of psychiatric hospitalization to transition into the community. Our mission expanded in 1988 to include homeless services and substance abuse treatment.

Conexio is seeking a motived, compassionate candidate to support the individuals in the capacity of a Registered Nurse. The Registered Nurse (RN) is a member of the clinical staff who provides medical expertise to the multidisciplinary teams responsible for the delivery of effective services to the individuals served to promote their health and wellness. This is a professional nursing position which requires thorough knowledge of commonly used psychotropic and health maintenance medications, an understanding of the special medical needs of adults with severe and persistent mental illness, substance abuse conditions or cognitive disabilities who may have co-occurring substance use disorders, physical disabilities, debilitating medical conditions, developmental disabilities, or infectious diseases. Must have the ability to function independently and as part of a team in carrying out the treatment plans and physicians’ orders in community settings.

Duties of the position:

  • Provides comprehensive case management services including coordination of employment, housing, mental health, addictions, medical, and financial services.
  • Maintains the clinical file for assigned clients
  • Participates in treatment planning and case conferences with other staff of the community support program and with other agencies.
  • Completes all assigned tasks from the daily schedule which may include the following:
    • Maintaining a therapeutic alliance with clients;
    • Referring and linking clients to all needed services provided outside the agency and following up to see that said services are delivered;
    • Coordinating the provision of emergency services and hospital liaison services when a client is in crisis;
    • Coordinating overall independent living assistance services and working with community agencies to develop needed resources including housing, employment options, and income assistance.
    • Providing support and consultation with client’s family;
    • Providing in-home services including ADL training, medication training, social support, and home management assistance.
    • Providing supportive counseling to clients.
  • Provides nursing care and gives guidance to consumers and staff in the management of consumers’ physical and mental health needs.
  • Manages the storage and administration of medications and treatments, ensuring compliance with applicable licensing regulations and statutes, including the Delaware Nurse Practice Act and Delaware Board of Pharmacy Regulations pertaining to Pharmaceutical Service in Health Care Facilities.
  • Maintains the primary relationship with the pharmaceutical contractor and assures compliance with all laws and regulations related to the storage, administration and disposal of medications.
  • Provides training and supervision to unlicensed personnel for the delivery of medical and personal care services to the residents in the areas of basic nutrition, observation and reporting of signs and symptoms of common diseases and conditions; assistance with hygiene and housekeeping tasks, vital signs measurement, catheter care, unsterile dressings and soaks, cold and heat applications, bowel and bladder training, incontinence care, use of assistive and prosthetic devices and care for dying patients.
  • Provides supervision for unlicensed personnel in assistance with self-medication.
  • Performs treatments and administers care in accordance with physician’s orders and protocols and/or as provided for by the standing orders or procedures of the facility.
  • Completes medical and physical assessments of all consumers at least annually.
  • Attends and participates in treatment team meetings, staff meetings, shift reports, training sessions and other meetings as designated by policy and/or as assigned.
  • Conducts symptom education and/or medication training, particularly as these relate to the communication, prevention, and treatment of infectious diseases.
  • Ensures that all routine and follow-up medical appointments are made, kept and documented.
  • Provides education and works as an advocate for the client in the community.
  • Provides nursing care and gives guidance to consumers and staff in the management of consumers’ physical and mental health needs.
  • Manages the implementation of the infectious disease control program, including prevention, screening and surveillance, treatment and hazardous waste management.
  • Provides training and supervision of unlicensed personnel in Advanced Directives, CPR, First Aid and procedures to be followed in life threatening and non-life threatening medical emergencies.
  • Performs treatments and administers care in accordance with physician’s orders and protocols and/or as provided for by the standing orders or procedures of the facility.
  • Documents and keeps other staff members informed of all changes in consumer mental status and physical health.
  • Assists and provides input to all team members, staff and physicians.
  • Ensures that all routine and follow-up medical appointments are made, kept and documented.
  • Communicates to other staff about medication changes and changes in consumer condition which may impact on behavior, treatment and/or restrictions.

Required Education and Qualifications:

  • Delaware Registered Nursing License in good standing or Compact License
  • Certification in First Aid and CPR
  • Valid driver’s license in good standing with at least 3 years minimum continuous driving experience with no serious traffic infractions.
  • Experience with electronic health records preferred
  • Excellent communication skills and organizational skills
  • Excellent ability to work with clients with substance abuse, co-occurring and/or mental health disorders
  • Excellent writing skills and record keeping skills
  • Detail oriented
  • Knowledge of addiction, substance abuse treatment services and counseling skills and theory

Additional Qualifications:

  • Computer knowledge of MS Office, and other HR applications is a plus, not required.
  • Possess strong verbal and written communication skills.
  • Must be proactive, organized, and possess great follow-up skills.
  • Must have the skill/ability to collaborate, partner with, and provide exceptional customer support to a variety of affiliate partners within the organization.
