The Certified Nursing Assistant (CNA) is a member the clinical team that provides compassionate care and assistance to persons who struggle with intellectual, developmental, and behavioral health challenges and require assistance to achieve their fullest potential. The CNA provides the assistance necessary to allow the clients we serve to live independently and ensure their wellness, health, and safety. The CNA, in collaboration with an interdisciplinary team, provides a wide range of services to meet the environmental, physical, psychological, and personal needs of the persons in our care. The CNA embraces the mission of organization by fostering a culture of compassion, hope, service, and collaboration. The Certified Nursing Assistant (CNA) is a trained professional who has direct contact with the consumer. The CNA is responsible for direct care and personal needs.

Duties of the position:

  • Ensures the effective delivery of clinical services in accordance with extant professional, ethical, and legal principles.
  • Conducts daily face to face visits with service recipients
  • Frequent contact with Core Team.
  • Supports and models the use of evidence-based practices
  • Ensures the maintenance of accurate and thorough electronic clinical records per Conexio requirements in a timely manner
  • Provide meal support to individuals
  • Prompt and assist individuals with brushing their teeth, dentures, gums
  • Provide support for bathing and dressing individuals
  • Assist individuals to and from the restroom and assist with changing soiled clothing
  • Provide ambulation assistance where needed
  • Take vitals; blood pressure, temperature, pulse, respiration, weight, blood sugar and administer medications
  • Transport individuals to and from medical appointments
  • Housekeeping and organization needed for individual’s bedroom areas
  • Communicate any change in mood, mental health status, vital signs, meal consumption, bowel movements, fluid intake, urine output, medication compliance and/or refusals to Residence Manager, Nurse, Nurse Manager and other staff
  • Document medical/occupational incidents on the appropriate forms
  • Document notes In Carelogic

Required Education and Qualifications:

  • High school diploma or equivalent
  • Delaware Certification as a Certified Nursing Assistant

Additional Qualifications:

  • Computer knowledge of MS Office, and other HR applications is a plus, not required.
  • Possess strong verbal and written communication skills.
  • Must be proactive, organized, and possess great follow-up skills.
  • Must have the skill/ability to collaborate, partner with, and provide exceptional customer support to a variety of affiliate partners within the organization.
  • 3 years of motor vehicle record and a valid DE drivers license
