Abraxas Youth & Family Services is hiring a dynamic Case Manager to join our community services PATH or Preparation for Adolescent Transition to Home program in Montgomery and Delaware Counties. Our team members are dedicated and passionate individuals that are committed to inspiring positive change in the lives of at-risk adolescents.
As a Case Manager, You Will:
Play an important role in assisting adjudicated youth transition out of the juvenile justice system.
In Summary:
The Case Manager is responsible for a caseload of adjudicated delinquent or dependent youth who have been court-ordered to residential placement or community-based programming. The position is responsible for successfully assisting the client (and the family) in transitioning from residential placement back to the community or working with youth directly referred from the community.
Salary: $23.00 per hour

At least twenty-one (21) years of age
Driver's License
Physical exam that includes TB and drug testing.
Criminal clearances (State Police, FBI and State Child Abuse Clearances)
Satisfactory completion of background screening and applicable pre-employment checks; including but not limited to employment and/or personal reference and driving records.
Ability to work with computers and the necessary software typically used by the department.
